For Individual Course Registration:
Complete this form. A check is required in advance in order to receive login instructions. Please mail to the address on the form: USDLC, P.O. Box 249, Flowery Branch, GA 30542. After processing, USDLC will provide you with course login information by email.

For Building Licenses:
Complete this form. A purchase order number is required on the form before faxing. FAX to 770-967-6568. After processing, USDLC will provide you with course login information by email.

(3) Please allow up to 24 hours for USDLC administrator approval upon successful submission. You will receive an email, subject line of USDLC Professional Development - Welcome which includes instructions on how to begin your coursework.
